Manchester score is an indicator of prognosis Prognosis is a medical term to describe the likely outcome of an illness. When applied to large populations, prognostic estimates can be very accurate: for example the statement "45% of patients with severe septic shock will die within 28 days" can be made with some confidence, because previous research found that this proportion of in small cell lung cancer Lung cancer is a disease of uncontrolled cell growth in tissues of the lung. This growth may lead to metastasis, which is the invasion of adjacent tissue and infiltration beyond the lungs. The vast majority of primary lung cancers are carcinomas of the lung, derived from epithelial cells. Lung cancer, the most common cause of cancer-related death.[1][2] It is calculated from a number of physical and biochemical markers.
A patient with small cell lung cancer scores one point for each of the following: -
- Serum Blood plasma is the yellow liquid component of blood, in which the blood cells in whole blood would normally be suspended. It makes up about 55% of the total blood volume. It is the intravascular fluid part of extracellular fluid. It is mostly water and contains dissolved proteins, glucose, clotting factors, mineral ions, hormones and carbon lactate dehydrogenase Lactate dehydrogenase is an enzyme (EC 1.1.1.27) present in a wide variety of organisms, including plants and animals exceeds the upper limit of the reference range.
- Serum sodium Sodium is a metallic element with a symbol Na (from Latin natrium or Arabic ناترون natrun; perhaps ultimately from Egyptian netjerj) and atomic number 11. It is a soft, silvery-white, highly reactive metal and is a member of the alkali metals within "group 1" (formerly known as ‘group IA’). It has only one stable isotope, 23Na concentration less than 132 mmol/l.
- Serum alkaline phosphatase Alkaline phosphatase (EC 3.1.3.1) is a hydrolase enzyme responsible for removing phosphate groups from many types of molecules, including nucleotides, proteins, and alkaloids. The process of removing the phosphate group is called dephosphorylation. As the name suggests, alkaline phosphatases are most effective in an alkaline environment. It is over one-and-a-half times the upper limit of the reference range.
- Serum bicarbonate In inorganic chemistry, bicarbonate is an intermediate form in the deprotonation of carbonic acid. Its chemical formula is HCO3− less than 24.
- Karnofsky performance status In medicine , performance status is an attempt to quantify cancer patients' general well-being. This measure is used to determine whether they can receive chemotherapy, whether dose adjustment is necessary, and as a measure for the required intensity of palliative care. It is also used in oncological randomized controlled trials as a measure of less than 60.
- Extensive stage disease.
Prognosis
| Manchester score | Prognostic group | Two year survival (%) |
|---|---|---|
| 0-1 | Good | 16.2 |
| 2-3 | Medium | 2.5 |
| 4 or more | Poor | 0 |
